How this list is built

Every report and reply is screened before publication. Contact details, abuse, and accusations naming private individuals are removed rather than published, and anything uncertain is held for a person to look at.

Reports are personal accounts, not verified facts. Treat a single report as one person's experience, and weigh it against how many others describe the same thing — and against whether an agency has complaints on file for the number.
